Of course, 'fon,' you may be right. But I'm going by my own experience, and by those of others. It is absurd to send a detailed form to someone who has recently come out of a psychiatric unit. Get vaccinated twice - and THEN test yourself twice a week?

This morning's BBC News website says that everyone is supposed to test themselves twice a week to see if they have coronavirus. Does this come on top of having been already vaccinated once or twice? Since these tests are bound to be highly unreliable, why should I risk being isolated 'just in case'?...
